Typical documentation for learning disabilities should include an educational or psychological evaluation report that was completed using adult norms. Current documentation is always preferred as it puts SDS in a better position to determine reasonable accommodations and support. Diagnostic reports should be completed by qualified professionals, and it is preferred that they include the following minimum requirements:
- A clear explanation of the diagnosis and/or functional impacts, which may include previously utilized accommodations or interventions (and their effectiveness)
- Scores from the tests administered
- Recommendations and rationale for reasonable academic and/or clinical accommodations
If you have any questions regarding learning disability documentation, please do not hesitate to contact SDS directly.
