Learning Disabilities

Typical documentation for learning disabilities should include an educational or psychological evaluation report that was completed using adult norms. Current documentation is always preferred as it puts SDS in a better position to determine reasonable accommodations and support. Diagnostic reports should be completed by qualified professionals, and it is preferred that they include the following minimum requirements:

  • A clear explanation of the diagnosis and/or functional impacts, which may include previously utilized accommodations or interventions (and their effectiveness)
  • Scores from the tests administered
  • Recommendations and rationale for reasonable academic and/or clinical accommodations

If you have any questions regarding learning disability documentation, please do not hesitate to contact SDS directly.
